Who to contact in case of an emergency

Release of Liability Waiver

In consideration of being allowed to participate in any camp, field trip, and/or program at The Leader, the undersigned, on his or her behalf, and on the behalf of the participant(s) identified below agrees that:

  • 1. I willingly assume the risks associated with participation and accept that there are also risks that may arise due to OTHER PARTICIPANTS, which I also willingly assume.
  • 2. I agree that I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any party and/or program at The Leader.

Media/Video Release:

I give The Leader permission to videotape/photograph/audiotape and or allow the videotaping, photographing and audio taping of my child. I will give written notice if not agreeing on this.

Media Camp Sign Up-Payment Policies

Media Camp costs $350.

  • 1. With the completion of the payment information section of this form, I allow The Leader to charge a $175, non refundable deposit to my card.
  • 2. I understand that the remaining $175 is due by July 9th.
  • 3. The Leader has the right to cancel a class or camp one week prior to the start date. Full refunds will be processed if a class/camp is cancelled.

Before and After Camp Care

Before and/or After camp care is one flat fee of $50.
Camp begins at 8:00 a.m. and ends at 5:00 p.m. Before camp care is from 7:15 a.m. – 7:50 a.m., and After camp care is from 5:10 p.m. – 6:00 p.m. I understand that if I bring the camper to camp before 7:50 a.m. or pick them up after 5:10 p.m. I will be charged $50.
